PPO and HMO self-employed insurance are options that include co-pays. Individuals pay certain amounts for every single one of the doctor visits. You would only be limited as to what doctors can be visited. Referrals are necessary when seeing specialists. The largest selection in this case is offered when you choose PPOs.
